Eligibility
Open to disabled individuals, ages 18 to 70, who:
- reside in the service area;
- are capable of directing their own services;
- are employed, preparing for employment, in school, or are actively volunteering in the community
Required documents
Completed pre-application and screening, proof of residency, proof of income, proof of program activity, physician's certification required, statement of agreement

Provides routine, non-medical assistance to adults with disabilities who are employed, involved in community volunteer work, or attending school and are not eligible for Medicaid. Personal assistants help with tasks such as: light house keeping, bathing, dressing, preparing meals, shopping, driving, or using public transportation.
