Eligibility

Adults 18 years or older who meet HUD low-income requirements; have a medically documented severe, chronic physical disability; are mentally alert, communicative, self-directed. In addition, all applicants must be able to self-evacuate in under three minutes and capable of spending 20 hours outside of the house involved in volunteer work, employment, or attending school/training.

Photo ID, medical clearance, reference from current case manager or personal-assistance provider, personal references from work/school/volunteer activities and family/friend, proof of income and assets

Application process

Call or visit the website to download, complete, and return the pre-application and residency application to the case manager.

Fees

Rent is income-based, approximately 30% of adjusted income.
