Provides ongoing care management services for residents with disabilities, ages 18 through 64 years of age. Services include advocate on the client's behalf and promote self-advocacy, assist with assessing the needs of consumer, connection to financial benefits programs, home health care, housing, meals on wheels, offer peer support, provide funding for adaptive, transportation equipment and home modifications.
