Eligibility
Open to adults, ages 18 and older, with a history of previous hospitalizations for a primary Axis I psychiatric diagnosis. In addition, applicants must be having difficulty in accessing services.

Delivers intensive outreach case management to adults with serious and persistent mental illness, especially those recently discharged from psychiatric hospitals. The goal is to decrease hospitalization frequency, extend community service connections, and includes engagement, assessment, service planning, medication monitoring, daily living skills assistance, housing support, educational/vocational services, service coordination, and advocacy. Participants are enrolled for a minimum of 12 months.

Offers the following Programs: Psychiatric and Mental Health Services, Individual, group and family counseling on a broad range of issues, Psychiatric Crisis Intervention, 24 hour emergency evaluation for the county, crisis stabilization, provided through the school system, Workshops for families of the mentally ill, individual and group counseling.
