Eligibility
Open to uninsured and low-income residents of the coverage area. Family income must be below 300% of the federal poverty guidelines.
Required documents
Government issued photo ID, proof of residency (bills or mail), and proof of household income (from all family in household with income), pay stubs, W-2s, Social Security documentation, pension/disability statements, unemployment documentation, benefit/award letters, and/or federal Form 1040.
