Provides housing solutions for adults with developmental or multiple physical disabilities. Housing options include: group homes, supported and supervised apartments, and independent living units. Each facility provides case management, personal assistance, nursing services, rehabilitation technology access, training in independent living skills, transportation access, community integration activities, fully accessible residences, building maintenance, certified home health care, and rent subsidies.
