Application process
Obtain a referral letter from a 501(c)(3) non-profit on a company letterhead with a Tax ID number (such as a school, church, Lions Club, Red Cross, United Way, etc.). Bring the required documents to a participating location (use the website to find the closest).
If consumer does not have an eyeglass prescription, they can inquire with a retail location about cost-free eye exam.
