Eligibility
Must be 18 years of age or older. Diagnosis of a severe and persistent mental illness in accordance with DSM IV TR criteria. Have been placed on CEPP status coming out of the hospital. Demonstrate capacity to function independently. Demonstrate ability to perform basic ADL's. Complete medical history. Satisfactorily complete a brief visit.
