Application process
Call to schedule an appointment. A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one.
Fees
Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. There is a prorated rate option available for uninsured clients. For Charity Care, call the hospital for more information on how to apply.

Provides diabetes self‑management education and training for individuals living with diabetes to support effective daily disease management. Offers individual and group education focused on building skills, setting goals, and improving health outcomes. Covers topics such as understanding diabetes and treatment options, nutrition and physical activity, medication use, blood‑glucose monitoring, prevention and management of complications, problem solving, psychosocial well‑being, and pregnancy‑related diabetes care when applicable.
