Application process
A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one, only if the patient has a current primary doctor.

Empowers individuals with diabetes through collaborative Diabetes Self-Management Education Classes (DSME) and training, guiding them to modify their behavior and successfully self-manage the disease. Facilitated by licensed health professionals, the course covers evidence-based topics such as the disease process, treatment options, nutrition management, physical activity, medication utilization, blood glucose monitoring, prevention and treatment of acute and chronic complications, goal setting, problem solving, psychosocial adjustment, and preconception care and management during pregnancy if applicable. Classes can be conducted individually or in groups.
