Application process

Call to schedule an appointment. A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one.

Fees

Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. There is a prorated rate option available for uninsured clients. Please call for information. For Charity Care, call the hospital for more information on how to apply.

A collaborative course through which people with diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage the disease and its related conditions. The course incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards. DSME can be conducted on an individual and/or group basis and is facilitated by a licensed health professional.
