Open to individuals ages 21 and older who have permanent or chronic disabilities or other debilitating illness.
Required documents
ID, Personal data forms, Recent physical exam & TB test (within 90 days), Immunization records, Guardian and medical consents, ISP, IEP, psychological/psychiatric assessments, Proof of guardianship, birth certificates, insurance cards, and Medication list and 10-day supply upon admission
