Eligibility
Residents of Gloucester County struggling with a mental illness, who meet any of the following:
- Adults currently in the state psychiatric hospital, county psychiatric hospital, or in a local psychiatric facility who could live in the community in an appropriate environment
- Adults in the community who have a history of hospitalization in state or local psychiatric facilities and are a risk of re-hospitalization
- Adults in the community who are mentally, emotionally, or functionally impaired and in need of daily living skills education to be about to live independently.individuals

Application/registration forms, birth certificate, psychiatric/psychological assessment, referral form.
