Combines intensive case management and recovery support services for individuals during pregnancy and up to one year after giving birth. Intensive case management focuses on developing a single, coordinated care plan for pregnant/postpartum individuals, their children, and families. Recovery support specialists provide non-clinical assistance and recovery supports.
