Eligibility
Open to adults that meet the following conditions:
- Diagnosis of substance abuse or dependence
- Minimal risk of physical withdrawal
- Psychiatrically stable
- Biomedical conditions that are manageable within this level of care
- Willingness to participate voluntarily in the program
Required documents
Birth Certificate, Social Security Card, ID, Judgment of Conviction for all prior CDS offenses.

Offers a wide range of outpatient services which may be coordinated by a case manager. This may include individual and group counseling, 12-step meetings, social and recreational activities, educational and vocational services, life skills training, relapse prevention services, a continuing care program and supportive services (such as childcare, transportation and parenting skills development).
