Eligibility

Open to adults that meet the following conditions:
  • Diagnosis of substance abuse or dependence
  • Minimal risk of physical withdrawal
  • Psychiatrically stable
  • Biomedical conditions that are manageable within this level of care
  • Willingness to participate voluntarily in the program

Required documents

Birth Certificate, Social Security Card, ID, Judgment of Conviction for all prior CDS offenses.

Fees

The program accepts Medicaid, Medicare, and self pay with a sliding scale fee system available. State/County funding and the Fee for Service Initiative (FFS) are also available.

Offers a wide range of outpatient services which may be coordinated by a case manager. This may include individual and group counseling, 12-step meetings, Social and recreational activities, life skills training, relapse prevention services, and continuing care program and supportive services (such as childcare, transportation and parenting skills development).
