Eligibility
The Tri-County RIST program accepts residents from Essex or Hudson County who are homeless and are currently on CEPP status at Trenton, Hagedorn, or Greystone Park Psychiatric Hospitals. Applicants must indicate a presence of a major DSM Axis I psychiatric illness and show a need for intensive support in order to maintain independent living.

Assists individuals with residential needs in transitioning from hospital to community living by securing permanent housing with state-subsidized rental assistance vouchers, developing wellness goals, and providing personal assistance for education and living skills. Offers outreach for mental health, substance abuse, and medical services. Coordinates with service providers, provides 24-hour staff availability, and supports peer and community social connections. Provides career counseling, job seeking skills training, and clinically enhanced support including a psychiatrist, wellness nurse, housing specialist, and Mental Illness and Chemical Abuse (MICA) and vocational counselors to prevent unnecessary hospitalizations and enhance quality of life.
