Required documents
Photo ID for child guardian, referral documentation, insurance or Medicaid information, clinical behavioral health records, court or DCP&P documents.

Mon - Fri 8:30 am - 5:00 pm
Application process
A referral is required from DCP&P to receive services.
Fees
Based on a sliding income scale.
