Call to schedule an appointment. A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one.
Fees

Delivers collaborative Diabetes Self-Management Education Classes (DSME) and training, empowering individuals to modify behavior and successfully self-manage diabetes. Tailored to individual and group settings, facilitated by licensed health professionals, the course addresses the diabetes disease process, treatment options, nutrition management, physical activity, medication utilization, blood glucose monitoring, acute and chronic complication prevention, goal setting, problem solving, psychosocial adjustment, and preconception care for those applicable. Guided by evidence-based standards, the program incorporates the unique needs, goals, and life experiences of each participant.

**Part of the Diabetes Initiative managed by the NJ Department of Health** DSME is for individuals who have been living with a Diabetes diagnosis for some time already, and/or want to be better educated on self-management techniques.
