Application process

Call the agency to apply for the program. A physician's certification is required. A social worker will make a home visit to assess the client's needs and appropriateness for the program. A waiting list may be in effect.

Offers seniors who are recovering from an illness or injury that may not require full-time medical assistance but are in need of some personal help with daily activities like bathing, laundry and housekeeping duties. Also offers residents short- term home health aide care at no cost.
