Eligibility

Open to individuals with a substance use and/or mental health disorder.

Required documents

Driver's license, DUI arrest report, financial statements, ID, Health Insurance card, proof of address, proof of benefits (i.e. EBT, TANF, GA), proof of income eligibility, psychiatric/psychological assessment, referral form, unemployment benefits statement.

Fees

Accepts Medicaid, private health insurance, and self pay with a sliding scale fee system available. State and County funding, DUII, and the Fee for Service Initiative (FFS) are also available.
