Eligibility
Eligible individuals are adults, ages 21 or older who are:
- In need of on-going skilled services of a nurse, social worker, physician assistant
- Adults who have a medical diagnosis that causes impairment but does not require 24-hour care in a hospital or nursing home.
- Adults who enjoy spending 5 hours per day in therapeutic recreation and social programs.

Application process
Contact the service provider to schedule an appointment for an assessment and tour of facility.
Fees
$95.00 per day, covered by Medicaid and long-term care insurance for qualified members; private pay also available.
