Open to youth up to 21, with behavioral, emotional, substance misuse issues and/or developmental or intellectual disabilities in the service area.

Bergen County, NJ
Provides community-based care management and Wraparound planning services for youth with serious emotional, behavioral, substance use, intellectual, or developmental challenges. Coordinates individualized Child and Family Team meetings, service planning, advocacy, crisis prevention, and linkage to community resources to help youth remain safely at home and in their communities. Behavioral Health Home services are available for eligible youth with co-occurring medical and behavioral health needs
