Description
Refers to the delivery of long-term care coverage through NJ Familycare for individuals who meet the clinical requirements for 24 hour nursing care. Under MLTSS, an individual's HMO will coordinate all healthcare services whether the individual resides in the community, in an assisted living facility, a residential facility or a skilled nursing facility.
