Eligibility

Open to individuals with a substance abuse addiction and/or mental health disorder.

Required documents

driver's license, DUI arrest report, financial statements, ID, ID with address, photo ID, proof of address, Proof of benefits (i.e. EBT, TANF, GA), proof of income eligibility, referral form, state ID, unemployment benefits statement

Fees

Accepts Medicaid, private health insurance, and self pay with a sliding scale fee system available. State and County funding, DUII, and the Fee for Service Initiative (FFS) are also available.

Provides a diverse array of outpatient services, overseen by a case manager. Additionally, treatment for co-occurring disorders includes mental health screenings, evaluations, diagnosis, medication management, and adherence monitoring.
