Eligibility

Open to adults, ages 18 or older, with a history of psychiatric hospital admissions for serious and persistent mental illness, impaired functioning who have been approved by the Division of Mental Health Services (DMHS). Include individuals with the following conditions:
  • Schizophrenia/other Psychotic Disorders
  • Major Depressive Disorders
  • Bipolar Disorders
  • Delusional Disorders
  • Schizoaffective Disorder

The program aims to reduce hospitalizations, enhance vocational opportunities, and improve the quality of life. PACT services are available lifelong if needed.
