Eligibility

Individuals age 18 and older, suffering from serious mental illness, or suffering from serious mental illness and from substance abuse, and who are homeless or at imminent risk of becoming homeless.

Offers community-based case management and outreach services for adults who are dealing with a serious and persistent mental illness and are homeless or are at risk of becoming homeless. This program also provides targeted case management services for military veterans experiencing homelessness and mental health issues.

Offers outpatient mental health services and substance abuse services, adult partial care, adolescent partial care, geriatric care, and residential services for adults and adolescents. A private non-profit community mental health center.
